Advanced Care Planning- What is it? How does it work? Why do I need to be concerned about doing this?
Advance care planning (ACP) is an ongoing process in which you, your loved ones and your health care team discuss your goals/values and how they affect your current and future medical care. This type of planning can include the creation of documents containing your medical care preferences, such as a living will or medical power of attorney.
We have partnered with a company called Iris to provide this service. We will be reaching out to patients to offer this service to some of our patients. If you are interested in having a discussion about the kind of care you would want to receive if you became very ill, we can connect you with Iris, or we can arrange an appointment for you to have that discussion here at our practice.

We want you to continue to “Stay Well at Home” during this time. We are still offering Telehealth visits and encourage you to utilize those as a standard these days for care when at all possible. We are seeing patients in the office. Our screening process at the door is allowing us to keep sick patients, patients with fever, and/or other possible Covid-19 symptoms from entering the building. Our providers are ready and prepared to see you in your vehicles if needed.
Our first priority is to our patients and our staff and ensuring safety of everyone involved.
If you come to the clinic and you have your own mask please wear it to help with the reduction in use of PPE (personal protective equipment). If you do not have a mask we will provide you with one to wear during your visit.
Please do not bring visitors, family or anyone with you to your appointments. Only patients, parents/guardians of minor children and caretakers will be allowed to enter the facility with patients. We need to ensure exposure to other patients and our staff is as limited as possible.
Please check your medications and let us know at least one week in advance if you need refills. We can review your chart and determine what needs to be done about getting refills.
During this time we will continue to be open Monday to Friday 7:30am to 5:00pm for all your healthcare needs. ...

FUN FRIDAY PIC!
This is not our entire crew but I think they represent well......these people are just some of the smiling faces you can NOT fully see during these times because of the use of masks in our everyday lives.
But i assure you they are smiling, here for you and ready to help in any way they can. We have a great team at Parekh Medical and we are so proud of their hard work and willingness to adapt during these times!

Beginning Monday May 18-2020 we are going to transition into being open Monday to Friday 7:30am-5:00pm.
As the world around us is easing into a sense of normalcy we are trying to do the same. As of right now we feel being here a few extra hours per day will help our patients!
We appreciate all your understanding and continued support as we make changes.
Just a few reminders:
We are still encouraging Telehealth or Phone visits at this time as a standard first choice option.
We will see you in the office after you are pre-triaged and determine the next steps in your care. You may be seen in the clinic or our provider may see you from your car.
If you need to make an appointment please call us at 662-562-8278 and we can do your pre-triage over the phone and give you an appointment time.
If you have to walk in you will be met by one of our staff members upon arrival to the clinic who will perform this process.
